North Vista Hospital maintains medical records for each patient treated at the hospital. To request a copy of your medical records, please contact our Health Information Management office between the hours of 8:00 a.m. to 4:30 p.m. at (702) 657-5533. We are closed for lunch between noon and 1:00 p.m. For information about birth certificates, call (702) 657-5532. If you have questions regarding release of information, please call (702) 657-5533.

Prior to receiving your records, you will be asked to complete a medical records release authorization form and to provide proof of your identification with a valid government issued photo ID.

Records needed for continuing care purposes will be sent directly to your provider.

Requests for medical records generally take up to 72 hours to process.
