North Vista Hospital maintains medical records for each patient treated at the hospital. To request a copy of your medical records, please contact our Health Information Management office between the hours of 8:00 a.m. to 4:30 p.m. at (702) 657-5533. Questions regarding release of information can also be answered by calling the same phone number.

Prior to receiving your records, you will be asked to complete a medical records release authorization form and to provide proof of your identification with a valid government issued photo ID.

Records needed for continuing care purposes will be sent directly to your provider.

Requests for medical records generally take up to 72 hours to process.

DESTRUCTION OF
HEALTH CARE RECORDS

Pursuant to Nevada Revised Statutes (NRS) 629.051:
1. The health care record of a person who is less than 23 years of age may not be destroyed.
2. The health care record of a person must be maintained for 5 years, after it has been received or created, unless federal law requires that it be retained for a longer period of time.
3. The health care record of a person who has reached the age of 23 years may be destroyed after 5 years from the date the record was received or created, unless federal law requires that it be retained for a longer period of time.
